I moved to UT 7/2024 from CA. My 1099R only has CA information in boxes 15-17. I was paid from Jul - Dec while I lived in UT and know I should pay taxes on that income. But, I’m at a loss on completing the TC-40W without information from boxes 15-17 and UT does not want any W2 or 1099s mailed to them? Would I complete as much as I can and submit with my return? Thank you.

From the [Utah TC-40W form]:1

Do not send your W-2s or 1099s with your return. Instead enter W-2 or 1099 information below, but only if there is Utah withholding on the form.

The TC-40W form is used to specify how much withholding was sent to Utah.

My 1099R only has CA information in boxes 15-17

That means that none was sent to Utah. A 1099 or W-2 is used to document how much tax money was withheld and where it went. The state lines say that for some or all of the checks you were a resident of state X, they have no idea if that is correct.

The rest of the Utah TC-40 and related forms are used to calculate how much money is taxable by Utah.

You moved during the year, so you will have to look at the part year tax forms for both states to determine how the split is done.
